Meaning, unpacked
Central idea
The film can be read as a study of letting go: it stages grief, guilt and the fear of dying as a bewildering descent in which clinging to a life, a person or a story is what causes suffering.
Its lasting influence
Its fusion of trauma, shifting reality and grim urban decay influenced later horror, notably the Silent Hill games and films built around a protagonist who is unaware of his own condition, such as The Sixth Sense and Shutter Island.

Jacob returns to his old family apartment and finds his son Gabe waiting. Together they climb a staircase into a bright light. We then see Jacob dead on a Vietnam field hospital table: he was dying all along, and the film was his passage out.
The last image
In a Vietnam field hospital, Jacob lies dead on a table.
What it says
The visions were Jacob's resistance to letting go of his son, his guilt and his life. Louis says the demons are attachments burning away, so the ending offers release, not a twist. The horror was refusing to accept loss.

An Occurrence at Owl Creek Bridge (1961)
A short film built on the same idea of stretched, subjective time. Seeing it after Jacob's Ladder shows how old the device is and how differently it can be used.
